PROCEDURES AND REQUIREMENTS FOR REPORTING THE DEATH OF FILIPINO CITIZENS ABROAD (Leia em Português)

The death of a Filipino citizen occurring in a place outside of the Philippines must be reported to the Embassy of the Philippines in order for the said death to be properly registered and recorded with the Office of the Civil Registrar General in Manila, and for other legal purposes.

The Embassy can only undertake the death registration process if the death occurred in countries within in its jurisdiction, i.e., Brazil, Colombia, Suriname and Guyana. For deaths that occurred outside of its jurisdiction, the Report of Death must be filed with the Philippine Embassy or Consulate that exercises jurisdiction over the place of death.

REQUIREMENTS FOR REGISTRATION OF DEATH AND THE ISSUANCE OF A CONSULAR MORTUARY CERTIFICATE (FOR SHIPMENT OF HUMAN REMAINS TO THE PHILIPPINES)

1. Accomplished Report of Death Form (Form No. 39). The person accomplishing the Report must ensure that all the details are completed to avoid unnecessary delays.

2. Authenticated photocopy of the Philippine passport of the deceased Filipino citizen, and other documents of identification such as Permanent Resident Card, Driver's License, etc.

3. Certificate of Death issued by the Local Civil Registrar or the local authority that issues Certificates of Deaths. If the Certificate is written in a language other than English, a corresponding official English translation from a Sworn Translator must be submitted together with the original Certificate. Translated documents must be duly notarized by the local Ministry of Foreign Affairs.

4. Duly notarized Certificate issued by the Mortuary/Funeral Director, stating that the body was properly embalmed/cremated in accordance with internationally accepted health practices; that it is in a sanitary condition consistent with existing standards for overseas shipment; and that the casket/urn has been hermetically sealed and contains only the remains of the deceased;

5. Certificate of Non-Contagious Disease issued by the proper health authority, stating that the deceased did not die of a contagious disease or that the remains to be shipped did not originate from a place contaminated by a contagious disease; and

6. Transit Permit issued by the proper health authority at the place where transportation begins.

In addition to the above-mentioned documents, information should also be provided to the Embassy, to include the following:

a) Name of Consignee/Addressee in the Philippines

b) Mode of shipment (By Air or Sea Transport)

c) Name of Carrier/Vessel

d) Date and Port of Embarkation

e) Port of Destination and Scheduled Time of Arrival in the Philippines

NOTES:

- For all documents written/executed in a language other than English, a corresponding official English translation done by a certified/sworn translator should be submitted together with the original or its certified true copy. Translated documents must be duly notarized by the local Ministry of External Relations.

- All the above documents must be submitted in original copies and must be submitted together with three photocopies each. The original documents will be returned after verification/evaluation of their authenticity.

- The Consular Officer may require other documentary requirements, if deemed necessary, after evaluation of all the documents submitted.
