Report Of Birth

When a child is born abroad to at least one Filipino parent, the child's birth must be immediately reported to the Philippine Embassy or Consulate which exercises jurisdiction over the place of birth.

Applications for Report of Birth after one (1) year from the birth of the child shall require the execution and submission of an Affidavit of Delayed Registration. 

 

A. FOR CHILDREN WHOSE PARENTS ARE MARRIED

Accomplish all forms completely. If possible, fill-up the forms electronically (on your PC). If the information requested is not applicable, write the letters N/A. Write legibly in capital/bold letters. Incomplete forms will be returned and will cause delay in the processing of the application.

Get your documents Apostilled (when applicable) by an authorized cartorio (in Brazil) or by the relevant authority in your country of residence usually the Ministry of Foreign Affairs.

Four (4) sets of duly accomplished REPORT OF BIRTH form signed by one or both parents. When the birth is reported in person, the Report of Birth will be sworn to before a Consular Officer.

2.

One (1) Original Apostilled copy + three (3) photocopies of the Certificate of Birth of the child issued by the Civil Registrar/Recorder at place of birth. 

If the Certificate of Birth is not in English, it must be translated by a certified/sworn translator. One (1) Original translation + three (3) photocopies of the translation should be submitted along with the Certificate of Birth.

3.

For parents who are married in the Philippines: Original and four (4) photocopies of PSA-issued Marriage Certificate.

*If the parents are not married, please consult Section B or C.

For parents who were married abroad whose marriage is reported at the Philippine Embassy/Consulate:

Original and four (4) photocopies of Marriage Certificate/Report of Marriage if birth of infant occurs within one (1) year from date of marriage;

Or

Original and four (4) photocopies of PSA-issued Marriage Certificate/Report of Marriage if birth of infant occurred after one (1) year from date of marriage.

5.

Four (4) clear photocopies of the data page of the current passport of both parents plus four (4) clear photocopies of the one or both Filipino parent’s valid Cédula de Identidade de Estrangeiros, popularly known as RNE or Protocolo if your RNE is still being processed (Brazil); MKV - Machtiging tot Kort Verblijf (Suriname); or the relevant Visa or Stay Permit issued by the country of your residence (Colombia, Guyana or Venezuela).

6.

Four (4) sets of duly accomplished Affidavit of Delayed Registration if report is made one (1) year after the birth of child. If the application is done by mail, the Affidavit must be Apostilled.

7.

Submit a self-addressed envelope and make mailing arrangements if you cannot pick up your documents personally. In Brazil, please arrange for *Vale Postal (R$20 for Carta Registrada or R$80 for SEDEX). Outside Brazil, please arrange for courier services.

B. FOR CHILDREN WHOSE PARENTS ARE NOT MARRIED BUT WILL USE THE FATHER'S SURNAME

Accomplish all forms completely. If possible, fill-up the forms electronically (on your PC). If the information requested is not applicable, write the letters N/A. Write legibly in capital/bold letters. Incomplete forms will be returned and will cause delay in the processing of the application.

Get your documents Apostilled (when applicable) by an authorized cartorio (in Brazil) or by the relevant authority in your country of residence usually the Ministry of Foreign Affairs.

4.

Four (4) sets of duly accomplished Affidavit to Use the Surname of the Father issued and signed by the mother. If the application is done by mail, the Affidavit must be Apostilled.

5.

Four (4) sets of *hand-written acknowledgment if the father is not a Filipino citizen. If in another language other than English, a corresponding official English translation (4 sets also) done by a certified/sworn translator should be submitted.

*Simply copy the text for the Affidavit of Acknowledgement/ Admission of Paternity.

If the application is done by mail, the Acknowledgment must be Apostilled.

If applied personally in the Embassy, a processing fee of R$131.25/US$25 is applied.

6.

C. FOR CHILDREN WHOSE PARENTS ARE NOT MARRIED AND WILL ADOPT THE MOTHER'S SURNAME

Accomplish all forms completely. If possible, fill-up the forms electronically (on your PC). If the information requested is not applicable, write the letters N/A. Write legibly in capital/bold letters. Incomplete forms will be returned and will cause delay in the processing of the application.
